Many of us have heard about the importance of having an advanced healthcare directive in order to share our important medical choices should we be unable to speak regarding our medical care, end of life issues, donation of organs, etc. We will provide a very simple form to address the most important questions, and, if you choose, help you complete it so you are prepared and protected.
