First, complete this Membership form and return it to Guest Services at our Healthy Aging Center along with your payment or call us at 415-456-9062.
You’ll receive a Vivalon keychain scan card to use for checking in at the front desk. This lets us know which services are important to you and ensures you receive your membership benefits.

Vivalon’s Healthy Aging Center and Jackson Café are friendly and inviting places for older adults to gather and participate in classes, activities and services.
