1. What matters most?
Talk about the kind of support your loved one would want if they needed extra help. Staying at home, maintaining independence, or living near family may all be priorities.
2. Who will help make decisions?
It’s important to know who will coordinate care, communicate with healthcare providers, and step in if needed.
3. What resources are available?
Many families aren’t aware of services like transportation, meal delivery, caregiver support, Medicare counseling, and in-home care until they need them. Learning what’s available ahead of time can reduce stress and expand your options.
4. Where is important information kept?
Make sure trusted family members know where to find medical information, insurance details, legal documents, and emergency contacts.
